MISSED APPOINTMENTS

We understand that your time is valuable and you can expect us to be prompt under normal circumstances.  We try our best to make appointment times convenient for you and your family.  We know you’re as busy as we are and we always try to accommodate your requests.  When appointments are broken without any notice, you not only affect the office, you also ruin the opportunity for other patients that may need that appointment time.  Please note that with this in mind we would like to inform you that when appointments are missed or broken without sufficient notice, then a $25 fee per ½ hour reserved time may be charged. 

 

INSURANCE PAYMENTS

Our office tries to go the extra mile in regards to helping you understand your insurance coverage.  We are glad to explain your coverage limits and do accept assignment of benefits as long as your plan permits.  However, we do not accept liability or guarantee of insurance payments.  If you are concerned about your out of pocket costs please ask our staff to prepare a written estimate for you to submit to your insurance company.  Please note we hold the patient ultimately responsible for payment of all services.  All deductibles, co-pays, and uncovered fees are to be paid at the time of services.  For your convenience we accept, Cash, Visa, Mastercard and Debit.  Due to past abuses we regret we can no longer accept personal cheques.  Please feel free to contact our office Manager should you need to make alternate payment arrangements prior to provision of any services.

As a community service, we do accept the following government plan:  ODSP and CINOT.  However, appointment restrictions do apply.  Please contact us for further information.
